Forms — Web App

From the web app, you can generate medical forms, review PDFs, print them, download them, and securely share them with a patient.

Access Forms

A patient's forms are available in the Forms tab of the patient record. You can also open a form from the Home activity feed when it appears in the list.

Generate a Form

  1. Open the patient record
  2. Click the Forms tab
  3. Click Form
  4. Select one or more form types
  5. Choose Generate, or Merge and generate when multiple forms should be combined

Generation runs in the background. The form can be opened when it is complete. A form that is still generating or in error cannot be selected for patient sharing.

Review, Print, and Download

Open a generated form to display the PDF in the review window.

In the window header, you can:

  • print the PDF;
  • download the PDF;
  • delete the form.

If you edit PDF fields, click Save before closing the window.

Send Forms to a Patient

You can share one or more generated forms directly with the patient.

  1. Open the patient record
  2. Go to the Forms tab
  3. Check the forms to share
  4. Click Send to patient
  5. Enter the patient's email address
  6. Click Send

The Send to patient button is disabled when no form is selected or when the patient has no date of birth. The date of birth is used as a verification step to protect access to the documents.

What the Patient Sees

The patient receives a secure link to the public documents page. To open the files, they must enter their date of birth in YYYY-MM-DD format. If the link is expired or invalid, the page displays a message saying that the link is expired. After the date is validated, the patient can download the shared documents.

Request a Missing Form

If the form you need is not available, click Can't find your form? from the form picker. You can upload a PDF and choose the target workplace. The Saro team will review the document and contact you when the integration is complete.
